The trapezius is a long, shallow muscle that extends vertically from the back of the neck down to the spine, and horizontally across to the shoulder blades. It moves the shoulder blades and supports the arms.
Although it plays an important role in upper body movement, an overdeveloped trapezius muscle can make the neck appear shorter or the upper body look broader. For women in particular, prominent trapezius muscles may affect the overall aesthetic balance.

Botox works by temporarily relaxing and reducing muscle activity. When Botox is injected into the trapezius muscle, the overdeveloped trapezius muscle decreases in volume, lowering the muscle height.
It also lowers the muscle profile, making the neck appear relatively longer and slimmer. As the collarbones become more defined, it creates a much slimmer and more feminine appearance.
